Advance Care Planning facilitator workshops
CAC will host two Respecting Choices Advance Care Planning Workshops on Saturday, October 7, 2017. First Steps training training is designed to help participants learn the skills needed to facilitate foundational ACP discussions with any adult. Last Steps training is designed to help participants learn the skills needed to have end-of-life discussions with frail elders or those living in long-term care facilities.
